Services Offered:

The professional or clinical services include:

o      Satellite pharmacies  located at the Operating Room, Delivery Room, Emergency Room

o      Oncology pharmacy including preparation of cytotoxic drugs

o      Unit dose drug distribution system

The business and administrative services includes:

o      Inpatient and Outpatient  Dispensing services

      Senior Citizen Discounts

o      Preparation of Total Parenteral Nutrition, other IV Admixtures and Compounded preparations

 

Senior Citizen Discount

1.    For Outpatients

1.1               The discount for the purchase of medicines shall be supported by the following requirements:

1.1.1             Senior Citizen’s identification document which can be any document or proof of being senior citizen which shall be used for the availment of benefits and privileges under the law. The following original documents can be used:

o      ID issued by the city or municipal mayor or Office of the Senior Citizen Affairs (OSCA) or of the barangay captain of the place where the senior citizen resides.

o      The passport of the elderly person or senior citizen concern.

o      Other documents that can establish the senior citizen or elderly person is a senior citizen of the republic who is at least sixty (60) years of age.

1.1.2             Physician’s Prescription with the following data:

o      Senior citizen’s name, age, sex and address

o      Prescription date

o      Generic Name of the medicine dosage form, dosage strength and quantity

o      Name and signature, address, contact number of the prescribing physician or dentist.

o      Professional license number, professional tax receipt (PTR)

o      And/or narcotic license number (S2 number)

Note: A doctor’s prescription is also required in the purchase of over-the-counter (OTC) medicines.

1.1.3             Purchase Booklet issued by the OSCA to senior citizen for free containing the following        information:

o      OSCA ID Number

o      Booklet control number

o      Date of Issue

o      Name of the senior citizen, sex, address, date of birth and signature

o      Information of the medicines purchased to be recorded in the purchased booklet.

1.1.4             Authorization letter of the senior citizen who is residing in the Philippines at the time of the purchase currently dated, and identification card of the representative, in case the medicine is bought by the representative of the senior citizen.

1.2               As a general rule, any single dispensing must be in accordance with the prescription issued by a physician and should not be more than one month supply.

1.3               For drugs with special discounted price less than 20% of the regular retail prices of drugs can deduct the percentage discount of the Promotional Campaign from the total 20% discount as required by R.A 9257. Thus, a total discount of 20% for senior citizens will still be observed.

Note: These discount privileges shall be non-transferable and exclusive for benefits of the senior citizen.

1.4               All medicines available in the pharmacy are subjected to this form of discount, galenicals, compounded prescriptions, and other miscellaneous items are exempted from the senior citizen discount.

1.5               Prices of medicines for senior citizen discount will be based on the outpatient price.

2.      For Inpatients

2.1               The discount for the charged medicines shall be claimed from the Billing Section upon discharge in accordance with the Patient’s Account Procedure.

2.1.1             Patient currently admitted and are in a cash basis status may avail of the discount from the Pharmacy Department as outpatient upon the procurement of the medications need. Purchases should be supported by the following requirements:

o      Senior Citizen’s Identification documents which can be any document or proof of being a senior citizen which shall be used for the benefits and privileges under the law. The Following original documents can be used:

      ID issued by the city or municipal mayor or Office of the Senior Citizen
Affairs (OSCA) or of the barangay captain of the place where the senior citizen resides.

      The passport of the elderly person or senior citizen concern.

      Other documents that can establish the senior or elderly person is a senior citizen of the Republic who is at least sixty (60) years of age.

2.2.            Fully accomplished, official MDH prescription coming from the nursing station where the patient is admitted.

2.3.            All medicines available in the pharmacy are subjected to this form of discount. galenicals, compounded prescriptions, and other miscellaneous items are exempted from the senior citizen discount

2.4.            Prices of medicines for Senior Citizen discount under this category will be based on the inpatients or charge price.
