Admission

Clients/patient should seek consultation with Physiatrist (medical practitioner specializing in the field of Rehabilitation Medicine) before initiation of therapy or treatment.

For Outpatients:

o      Patients under HMO are scheduled for consultation with affiliated Physiatrist.

o      Cash-basis patients are scheduled for consultation with Physiatrist of choice.

o      Direct referrals to Physiatrist (with referral letter) are scheduled accordingly.

o      In cases where patients opt to be seen by another Physiatrist, approval of referring doctor is secured first prior to scheduling of patient with Physiatrist of choice.

o      A consultation Appointment Slip is given to the patient/relative to remind of the pre-scheduled physiatrist consultation.

o      Patients under HMO are instructed to secure a Consultation Referral from company/
Insurance provider to be presented on the day of consultation.

o      Patients are advised to bring pertinent diagnostic/laboratory reports (e.g. X-ray) on the day of consultation.

o      On the day of Physiatrist consultation appointment:

      Patients under HMO asked to submit their Consultation Referral prior to consultation with physiatrist. The Following policy shall be followed” “No Referral Form. No Consultation Policy.”

      Cash patients settle their account after consultation.

      Patient/Relative fills-out pertinent data.

      Patient is seen by the Physiatrist for Consultation. The physiatrist orders the therapy sessions he deems necessary.

For Inpatients:

Patients are received in the form of referrals for Rehabilitation Medicine Services. The Rehabilitation Medicine staff ensures prompt facilitation of the referral to ensure consultation of the referred Physiatrist orders the therapy sessions he deems necessary.

After consultation with the Physiatrist and with the recommendation to undergo therapy (Physical Therapy / Occupational Therapy, Speech Therapy) the Rehabilitation Medicine staff makes an appointment with the patient based on the Physiatrist’s prescribed frequency and shall issue an OT/PT/ST Appointment Slip to the patient/relative. Therapy may be started immediately (on the day of consultation) if the therapist is available. If not, the patient is instructed to come back on the pre-arranged appointment date and time.

Therapy treatment account settlement will either be cash basis or charged to HMO.

o      For cash basis patients, the department secretary asses and inform the patient on cost of treatment per session. Accounts are settled after commencement of therapy session.

o      For patients under HMO, written requests for OT/PT Services are issued indicating the prescribed number sessions. Patients are instructed to secure Letter of Authorization (LOA) from HMO coordinator by presenting said request. Patients must submit their LOA prior to treatment. “No LOA, No Treatment Policy” shall be followed.
