Aking nauunawaan na ang Subscription Agreement ay base sa mga sagot ko sa itaas, na pawang totoo at kumpleto. Ako din ay pumapayag na ang Subscription Agreement ay pormal na magsisimula sa oras na ako ay makapag-bayad ng buo ng initial Subscription Fee, evaluation of my subscription and its issuance. I further certify that the Family Member(s) enrolled herein are all in good health as of the date of my completion of this Subscription Form. In case any of my enrolled Family Member is not in good health as defined in this Subscription Form, I, as Subscriber will not be entitled to the enumerated Program Benefits in case said Family Member passes away or is diagnosed of terminal illness during the effectivity of the Subscription Agreement.
SUBSCRIPTION TERMS and CONDITIONS I . PROGRAM BENEFITS Manor Management Corporation shall provide the Subscriber the following benefits: 1. Basic Subscription Benefits 1. If the enrolled Family Member passes away, i.. Round trip airline ticket (economy class) from the international airport of the Subscriber’s country of work or residence to the domestic airport nearest his Philippine residence indicated in his Subscription Form. ii.. Cash assistance equivalent to One Hundred Thousand (PhP 100,000.00) Pesos upon submission of the necessary documents. iii. Travel assistance that will enable the Subscriber to visit the Philippines. 2. Comprehensive Subscription Benefits 1. If the enrolled Family Member passes away, i.. Round trip airline ticket (economy class) from the international airport of the Subscriber’s country of work or residence to the domestic airport nearest his Philippine residence indicated in his Subscription Form. ii.. Cash assistance equivalent to Two Hundred Thousand (PhP 200,000.00) Pesos upon submission of the necessary documents. iii. Travel assistance that will enable the Subscriber to visit the Philippines. 2. If the enrolled Family Member is diagnosed with terminal illness before reaching age 60 and there is a request for advance payment which has been approved. i.. Round trip airline ticket (economy class) from the international airport of the Subscriber’s country of work or residence to the domestic airport nearest his Philippine residence indicated in his Subscription Form. ii.. Advance payment of Fifty Thousand Pesos (PhP 50,000) iii. Travel assistance that will enable the Subscriber to visit the Philippines. II EFFECTIVITY AND LIMITATION OF COVERAGE The Subscription Agreement shall be effective upon full payment of the Subscription Fee and upon acceptance of the enrolled Family Member(s) as enumerated herein. In case any of the enrolled Family Member is not in good health as defined herein as of Subscription Date, the Subscriber will not be entitled to the Program Benefits in case said Family Member passes away or is diagnosed with terminal illness during the effectivity of the Subscription Agreement. Good health means that the enrolled Family Member is actively at work or is pursuing his daily chores as of Subscription Date and has not over the last 5 years consulted a medical practitioner and has not been diagnosed for any heart condition, high blood pressure, cancer, diabetes, lung, kidney or stomach disorder, liver cirrhosis/jaundice, disorder of the thyroid gland or any other similar physical impairment. Terminal illness shall mean an illness of the enrolled Family Member where there is a life expectancy of 12 months or less from the date of the diagnosis of such illness and duly certified by a physician acceptable to Manor. This benefit is terminated at the anniversary date after the 60th birthday of the enrolled Family Member. It is hereby understood that only those qualified Family Members named in the Subscription Agreement will be covered. III. TERMINATION OF SUBSCRIPTION AGREEMENT The Subscription Agreement shall be automatically terminated at the end of term period of twelve (12) months from the date of the Subscription Agreement unless the Subscription Agreement is renewed on or before the termination date. In the event that the Subscriber predeceased any of the enrolled Family Members, the Subscription Agreement will continue to be in force unless terminated under the immediately preceding paragraph. The money value of the round-trip economy ticket and the Two Hundred Thousand (Php P200,000) Pesos Cash Assistance under Comprehensive Subscription or One Hundred Thousand (Php100,000) Pesos Cash assistance under Basic Subscription will be paid to the next of kin of the deceased enrolled Family Member upon receipt of Subscriber’s proof of death and the submission and approval of the necessary documents to support the death of the enrolled Family Member. On Membership Rules Enrollment Rules: 1. Family members qualified for enrollment: o Spouse – below 65 years old o Children – age 1 but below 21 years old (for family rate). Children above 21 years old are acceptable on individual rate o Parents – below 65 years old o Siblings – age 1 but below 65 o Other family members – age 1 but below 65 2. Family Members for enrollment MUST be in good health as of Subscription Date. 3. Family Members for enrollment MUST be residing in the Philippines as of Subscription Date. However, family members residing in the following areas – Sulu, Marawi, Tawi-tawi, Maguindanao and Lanao del sur DO NOT QUALIFY for Family Ties. 4. Subscription Rate guidelines: 1. Family Rate – to be used for the spouse and at least 2 children below 21 years old. If a family is composed of the spouse and one child only, it is cheaper to enroll using the individual rate. 2. If Overseas Pinoy-Subscriber is separated or single parent and hast at least 2 children below 21 years old, he/she can avail of the family rate by enrolling any one parent or sibling as replacement for the purpose. 3. Additional Child – to be used if there are more than 4 children being enrolled below 21 years old On Program Types and Benefits Basic Subscription Benefits 1.
