Membership Rates**:

Note:
Please refer to Manor the following:
1.Countries that are not reflected in the schedule before using any of the above membership fees.
2. Any relative or person to be enrolled that are not shown in the schedule.

Family rate - to be used for the spouse and at least 2 children. Otherwise, it is cheaper to enroll the spouse and child individually
Additional Child rate - is used if there are more than 4 children being enrolled.

Effectivity of Membership - effective for one (1) year. Membership fee for the following
year shall be paid on or before the end of the current year.

Billling Notice - a reminder notice of the membership fee due is sent to the subscriber 30 days
before the due date.

Payment Procedures

Upon completion of the Subscription Form, Subscriber has the option to pay through:
- Banks ( Bank of Commerce - Main Branch )
  - Accomplish a bank payment slip and indicate the following information:
    Account Name - RxPinoy
    Bank Current Account No. - CA-900-0000-1078-3
    Payment intention - Family Ties ( indicate name of OFW-subscriber)
Scan the remittance slip together with the Subscription Form
Send the Document file to us through email (help@rxpinoy.com).
Leave a file copy of the subscription form and remittance slip for future reference.
All subscription forms must have the remittance slip attached for our processing.
